The Agency for Healthcare Research and Quality (AHRQ) publishes SOPS surveys to help healthcare organizations to assess their culture’s support for patient safety.

AHRQ has surveys for hospitals, medical offices, nursing homes, community pharmacies, and ambulatory surgery centers.
