Getting ready to conduct a SOPS survey

When you partner with PSG to conduct a culture of safety survey, we'll have your survey ready to go in just a couple of days.

Here's what to expect during the setup process:

  1. Choose which AHRQ survey you want to deliver. You can conduct multiple surveys at no extra cost.
  2. Upload a list of facilities and department to be surveyed.
  3. Add any custom questions that you want to include.

That's it. You are ready to start your survey. The results will be available as soon as your survey is complete.

AHRQ Surveys on Patient Safety Culture™ (SOPS®)

The Agency for Healthcare Research and Quality (AHRQ) publishes SOPS surveys to help healthcare organizations to assess their culture’s support for patient safety.

PSG offers the following surveys:

  • Hospital safety culture.
  • Medical office safety culture.
  • Nursing home safety culture.
  • Ambulatory surgery center safety culture.
  • Community pharmacy safety culture.

In addition to the AHRQ surveys listed above, we also offer a safety culture survey for Home Health and Hospice. This survey was adapted by PSG in collaboration with a customer and is not recognized by AHRQ.

Our surveys are quick, simple to use, and mobile friendly.

Flexible, easy to use surveys

PSG surveys are designed with mobile in mind.

It can be difficult to get busy clinicians to sit down for 15 minutes and take a survey. We've found that response rates improve dramatically when surveys are available on cellphones and tablets as well as desktop computers.

Customize the survey for your organization

We've made it easy to customize the SOPS survey and add additional questions to get a deeper understanding of the cultural issues that are important to your staff.

For example, some organizations ask about employee safety as well as patient safety or ask additional demographic questions to give deeper insight when analyzing the survey results.

Note that if you plan to submit your survey results to AHRQ, you are only allowed to add questions to the end of the survey.

Keep track of responses

After you survey starts, you'll have access to a dashboard that shows how many employees have completed your survey on each day. If you tell us how many employees you expect to take the survey, we'll show you the response rate too.

With the right incentives, you can achieve response rates of 90%.

A participation report shows responses by department to help you can identify groups that might need a little encouragement.

Personalized invitations

Many customers prefer to send a single invitation to all of their staff. This allows staff to respond to the survey anonymously and is simpler to set up.

Alternatively, you can upload a list of staff to PSG and we'll send a personalized invitation to everyone who needs to take the survey. Responses will remain confidential and will only seen by PSG.

Personalized invitations allow you to track responses by department and to send individual reminders to anyone who forgot to compete the survey.

Personalized invitations usually result in a better response rate.

Diagnose your culture

Our customizable reports highlight key strengths to all of your internal stakeholders.

The PSG safety report shows your score broken down by safety category. Scores are calculated by counting the number of positive answers to each question and averaging the scores for each category.

Compare your results to national benchmarks and track your progress over time.

Using the safety report, you can

  • compare your scores to a national benchmark.
  • compare your scores to a previous survey.
  • see scores for individual facilities and departments.
  • filter results by role and work area.

Safety categories are color-coded and help identify areas for improvement.

You can drill down to see the results for individual survey questions.

Get a bird's eye view of your safety culture

Our matrix report provides an overview that helps you to identify departments with room for improvement.

Leaders can only see results for organizations that they are responsible for.

Color-coding highlights the categories that need attention across your whole organization.

Listen to your colleagues

The best recommendations often come from feedback your colleagues take the time to write down. All our surveys include free text comments.

PSG has a process to help you redact comments to maintain staff anonymity.

You can annotate comments with tags to group feedback by area of concern.

Analyzing comments helps incorporate your organization's best ideas into an action plan, creating a collaborative feedback loop. With PSG, everyday health care heroes know their input matters and that they are part of the solution.
